Error when reading data from an Excel book with Read from Sheet operation

Question

I tried to read an Excel book using Read from Sheet operation, but the following error occurred with "EXCEL0004E".
What is the cause and how can I fix it?

Processing of [excel_sheet(read from file/excel/sheet)] failed. Cause: [com.appresso.ds.dp.modules.adapter.excel.ooxml.api.zip.ZipEntryNotFoundException: entry with specified name did not exist in the zip file. name=xl/ workbook.xml]

Possible Cause
The specified Excel book may have a password.
Read from Sheet operation cannot specify a book with a read password.

Remove the password and specify the Excel book again.
